The Wellness After Breast Cancer Transition Program is a comprehensive, evidencebased initiative designed to empower women and men who have completed primary treatment (surgery, chemotherapy, radiation, or targeted therapy) to reclaim health, confidence, and quality of life. This program is not a onesizefitsall solution; it integrates personalized care plans, peer support, and expertled education to address the unique challenges each survivor faces. Each participant completes a thorough evaluation covering physical fitness, nutrition status, psychosocial wellbeing, and lymphedema risk. A certified survivorship nurse interprets the results and creates a tailored action plan. Registered dietitians provide survivorfocused meal planning, focusing on antiinflammatory foods, adequate protein, and calcium/vitamin D for bone preservation. Specialized guidance is offered for managing treatmentrelated side effects such as taste changes, nausea, or digestive issues. Certified lymphedema therapists teach riskreduction techniques, selfmonitoring, and compressive therapy options. Early detection can prevent chronic swelling and improve comfort. Participants are paired with alumni who have successfully navigated the transition. Mentors share practical tips, celebrate milestones, and provide a compassionate sounding board. Monthly checkins via telehealth or inperson visits track progress, adjust plans, and reinforce accountability. Data are entered into a secure survivor portal where participants can view trends and set new goals. Research consistently demonstrates that structured survivorship programs improve both objective health metrics and subjective quality of life.
