Tuberculosis: A Global Health Perspective

Tuberculosis (TB) is an infectious disease that primarily affects the lungs, though it can spread to other parts of the body, including the brain, kidneys, and spine. It is caused by a bacterium known as Mycobacterium tuberculosis. Despite being a preventable and curable disease, TB remains one of the world's top infectious killers, posing a significant challenge to global public health systems.

How Tuberculosis Spreads

Tuberculosis is an airborne disease. People nearby who inhale these droplets can become infected. It is important to note that TB is not spread through surfaces, such as shaking hands, sharing food, or touching bed linens. The risk of transmission is highest in crowded, poorly ventilated environments.

Latent TB vs. Active TB Disease

Not everyone who becomes infected with the TB bacteria develops active symptoms. Medical professionals distinguish between two main conditions:

  • Latent TB Infection: In this state, the bacteria live in the body but remain inactive. The person does not feel sick, has no symptoms, and is not infectious to others. However, the infection can become active later if the immune system is weakened.
  • Active TB Disease: This occurs when the immune system cannot contain the bacteria, allowing them to multiply. The individual experiences symptoms and can transmit the disease to others.

Symptoms and Diagnosis

The symptoms of active TB depend on where the bacteria are growing in the body. Symptoms of pulmonary (lung) TB often include a persistent cough that lasts for three weeks or longer, chest pain, and coughing up blood or sputum. Systemic symptoms may also include fatigue, unintentional weight loss, loss of appetite, chills, fever, and night sweats.

Diagnosis typically involves a combination of medical history, physical examinations, and diagnostic testing. Common methods include the Mantoux tuberculin skin test, blood tests (IGRA), chest X-rays, and laboratory analysis of sputum samples to identify the presence of the bacteria.

Treatment and Prevention

Tuberculosis is treatable with a course of antibiotics, usually taken for six to nine months. It is vital that patients complete the entire course of medication as prescribed, even if symptoms improve early on. Stopping treatment prematurely can lead to the development of drug-resistant strains of TB, which are much harder and more expensive to treat.

Prevention strategies focus on early detection, treatment of latent infections to prevent progression to active disease, and improving public health infrastructure. In some countries, the BCG (Bacille Calmette-Gurin) vaccine is used, primarily to protect infants and young children from severe forms of the disease.

The Global Impact

The fight against Tuberculosis is a central pillar of the United Nations Sustainable Development Goals. While significant progress has been made in reducing mortality rates over the last few decades, health organizations continue to work toward ending the epidemic. Addressing social determinants, such as malnutrition, poverty, and lack of access to healthcare, remains essential in controlling the spread of the bacteria and ensuring that those affected have access to the care they need.
