SAMHSAs Concept of Trauma and Guidance for a Trauma-Informed Approach

The Substance Abuse and Mental Health Services Administration (SAMHSA) has developed a comprehensive framework to assist organizations and systems in becoming trauma-informed. This guidance is essential for improving outcomes for individuals who have experienced trauma, which is widespread across all demographics and often intersects with behavioral health challenges.

The Three Es of Trauma

SAMHSA defines trauma through the "Three E's," which help practitioners understand the scope of the individual's experience:

  • Events: Trauma results from an event, series of events, or set of circumstances that is experienced by an individual as physically or emotionally harmful or life-threatening.
  • Experience: The individuals experience of these events determines whether it is traumatic. How the individual labels, assigns meaning to, and is affected by the event is subjective and personal.
  • Effects: The trauma has lasting adverse effects on the individuals functioning and mental, physical, social, emotional, or spiritual well-being.
The Goal: A trauma-informed approach is not just about identifying trauma; it is about shifting organizational culture from asking "What is wrong with you?" to "What happened to you?"

The Six Key Principles of a Trauma-Informed Approach

SAMHSA outlines six core principles that should guide any organization seeking to implement a trauma-informed approach:

  1. Safety: Throughout the organization, staff and the people they serve feel physically and psychologically safe. The setting is welcoming and helpful.
  2. Trustworthiness and Transparency: Organizational operations and decisions are conducted with transparency with the goal of building and maintaining trust among staff, clients, and family members.
  3. Peer Support: Peer support and mutual self-help are key vehicles for establishing safety and hope, building trust, and utilizing lived experience to promote recovery.
  4. Collaboration and Mutuality: There is a recognition that healing happens in relationships and in the meaningful sharing of power and decision-making.
  5. Empowerment, Voice, and Choice: Throughout the organization, individuals are supported in shared decision-making, choice, and goal setting to determine the plan of action they need to heal.
  6. Cultural, Historical, and Gender Issues: The organization actively moves past cultural stereotypes and biases, offers gender-responsive services, leverages the healing value of traditional cultural connections, and recognizes and addresses historical trauma.

Implementing the Approach

Moving toward a trauma-informed system requires a multifaceted strategy. It involves more than just staff training; it requires organizational change in governance, leadership, and physical environments. Leaders must commit to trauma-informed policies, ensure that the workforce is educated on the impact of trauma, and design service delivery systems that avoid re-traumatization.

By adopting these principles, organizations can create environments where individuals feel empowered to seek help and engage in the recovery process. This approach is instrumental in reducing the prevalence of behavioral health crises and ensuring that support systems are truly effective, compassionate, and sustainable.
