Before beginning, ensure you have correctly identified the patient following the two-identifier protocol (e.g., name and date of birth). Explain the procedure to the patient to reduce anxiety and gain consent. It is important to ask about any allergies, bleeding disorders, or previous complications with blood draws. Confirm if the patient has followed any pre-test instructions such as fasting.
Positioning the patient comfortably, usually seated or lying down, with the arm extended on a flat surface is important. This helps stabilize the arm and reduces the risk of fainting.
Begin by washing your hands thoroughly with soap and water or using an alcohol-based hand rub. Put on disposable gloves before touching the patient or equipment.
Place the tourniquet approximately 3 to 4 inches above the intended puncture site. It should be tight enough to restrict venous blood flow but not arterial flow. This causes veins to dilate and become more palpable and visible.
The preferred site is the antecubital fossa (front of the elbow). Common veins used include:
Palpate for a suitable vein. It should feel spongy, resilient, and without surrounding tenderness.
Use an antiseptic swab to clean the puncture site by wiping in a circular motion from the center outward. Allow the site to air dry completely to reduce the risk of hemolysis and prevent contamination.
Hold the patient's arm steady by placing your thumb below the puncture site to anchor the vein. Remove the needle cap and hold the needle bevel facing up.
Insert the needle smoothly at about a 15 to 30-degree angle with the bevel up. Once you feel the vein puncture (a slight "give"), advance the needle slightly to ensure it is within the lumen.
If using a vacutainer system, attach the appropriate collection tube onto the needle holder. Blood will begin to flow into the tube by vacuum suction. Fill tubes in the correct order of draw to prevent cross-contamination of additives.
If using a syringe, gently pull back the plunger to draw blood slowly.
Release the tourniquet once blood flow is established or before withdrawing the needle to reduce patient discomfort and risk of hemoconcentration.
Once the required amount of blood has been collected, withdraw the needle smoothly at the same angle it was inserted. Immediately apply gauze to the puncture site with firm pressure to prevent bleeding and hematoma formation.
Ask the patient to hold the gauze in place while applying an adhesive bandage once bleeding stops.
Activate any safety device on the needle and discard it into a sharps container. Dispose of used materials as per your facility's infection control protocols.
Label all collected tubes immediately at the bedside with patient identifiers, date, and time. Document the procedure in the patient's record, noting any difficulties or complications.
Veins may be smaller and more fragile in children and elderly patients, requiring smaller gauge needles and gentle technique. Use appropriate distraction methods for children to minimize distress.
Some patients, such as those who are obese, dehydrated, or have a history of multiple blood draws, may have veins that are challenging to locate or puncture. Alternative sites like the dorsal hand veins or the use of a butterfly needle with syringe may be necessary.
Strict adherence to aseptic technique prevents infection. Use gloves, avoid touching the puncture site after cleaning, and use safety-engineered devices to minimize needle-stick injuries.
Observe the patient for any immediate reactions, such as bleeding, swelling, or dizziness. Encourage the patient to keep the arm extended and avoid heavy lifting for some time. Advise the patient to report any delayed pain, swelling, or signs of infection at the puncture site.
