Peripheral Intravenous Cannulation: Clinical Guidelines
Peripheral Intravenous Cannulation (PIVC) is one of the most common invasive medical procedures performed in healthcare settings. It involves the insertion of a flexible plastic tube (cannula) into a peripheral vein to provide direct access for the administration of fluids, medications, blood products, and nutritional support. While routine, the procedure requires precision to ensure patient safety and minimize complications.
Indications for PIVC
The decision to insert a PIVC is based on the necessity for reliable vascular access. Primary indications include:
- Administration of intravenous fluids for resuscitation or maintenance.
- Delivery of intermittent or continuous medications (e.g., antibiotics, analgesics).
- Administration of blood or blood products.
- Frequent sampling for diagnostic blood tests.
- Prophylactic access for patients at risk of deterioration or requiring emergency intervention.
Site Selection
Choosing the appropriate site is critical for the longevity of the cannula and the comfort of the patient. The general rule is to start distally (further away from the heart) and move proximally (towards the heart) if a second attempt is needed. Preferred sites include the dorsal aspect of the hand and the forearm. The antecubital fossa should be avoided whenever possible, as it is a site of joint flexion which increases the risk of mechanical phlebitis and occlusion.
Procedure Steps
- Preparation: Gather all equipment, including the cannula, tourniquet, antiseptic wipes, sterile dressing, and flush solution. Verify the patient's identity.
- Site Assessment: Inspect for signs of infection, existing wounds, or compromised circulation.
- Application of Tourniquet: Apply 510 cm above the intended site to engorge the vein. Ensure it is not too tight to avoid arterial occlusion.
- Antisepsis: Clean the skin using an approved antiseptic solution (such as chlorhexidine) and allow it to air-dry completely.
- Insertion: Stabilize the vein by applying gentle traction on the skin. Insert the needle at a 1530 degree angle until blood flashback is observed in the cannula chamber.
- Advancement: Once flashback occurs, lower the angle and advance the cannula off the needle into the vein.
- Securing: Release the tourniquet, remove the needle, and secure the cannula with a sterile transparent dressing.
Complications and Management
Despite careful technique, complications can occur. Clinicians must monitor for the following:
- Infiltration: The leakage of non-vesicant fluid into the surrounding tissue. Signs include swelling, coolness at the site, and tenderness.
- Extravasation: The leakage of vesicant (tissue-damaging) drugs into the tissue. This is a medical emergency requiring immediate cessation of infusion and specific pharmacological intervention.
- Phlebitis: Inflammation of the vein wall. It may be mechanical (caused by the cannula), chemical (caused by the medication), or bacterial (caused by poor aseptic technique).
- Infection: Systemic or local infection at the site. Strict adherence to hand hygiene is the primary prevention strategy.
Maintenance and Documentation
The PIVC site should be assessed regularly, ideally once per shift or according to local institutional policy. Documentation should include the date and time of insertion, the size of the cannula, the site used, and the name of the clinician who performed the procedure. If a PIVC is no longer clinically indicated, it should be removed promptly to reduce the risk of bloodstream infections.
