What Is a Family Health Team?

A Family Health Team (FHT) is a multidisciplinary primary care model that brings together physicians, nurse practitioners, nurses, social workers, dietitians, and other health professionals to provide comprehensive , patientcentred care. The team works collaboratively to manage chronic conditions, promote preventive health, and coordinate services across the health system.

About North Shore Family Health Team

North Shore Family Health Team (NSFHT) serves residents of the North Shore region of Toronto, offering accessible, highquality primary care to individuals and families of all ages. Established in 2004, the team has grown to include more than 30 health care providers and a support staff dedicated to improving the health and wellbeing of the community.

The core philosophy of NSFHT is to treat each patient as a whole personnot just a list of symptoms. By integrating mental health, chronic disease management, and health promotion into everyday practice, the team helps patients achieve better health outcomes and a higher quality of life.
