Biphasic Liquids (Dispersed Systems)

In these systems, the drug is not fully dissolved but is either suspended as solid particles or emulsified as liquid droplets within a vehicle. These preparations are generally homogenous only when shaken.

  • Suspensions: Preparations containing finely divided insoluble drug particles distributed uniformly in a liquid vehicle. Suspensions must be shaken before use to ensure the correct dosage is administered. They are useful for drugs that are unstable in solution or have an unpleasant taste when dissolved.
  • Emulsions: Biphasic systems consisting of two immiscible liquids, usually oil and water. One liquid is dispersed as small globules within the other. Stabilizing agents called emulsifiers are required to prevent the globules from coalescing. Emulsions are often used to deliver oils or fat-soluble vitamins.

Advantages and Disadvantages of Liquid Dosage Forms

Advantages:

  • Better absorption and faster onset of action compared to some solid forms.
  • Flexibility in dosing; the dose can be easily adjusted by measuring volume.
  • Ideal for patients with dysphagia (difficulty swallowing).
  • Uniform distribution of the drug, reducing the risk of dose dumping.

Disadvantages:

  • Bulkier and heavier to transport and store.
  • Shorter shelf life due to potential microbial contamination or hydrolysis of the drug.
  • Need for precise measuring devices (cups, spoons) to ensure accurate dosing.
  • Some ingredients may precipitate over time, affecting dosage accuracy.

Semisolid Dosage Forms

Semisolid dosage forms are intended for application to the skin or mucous membranes. They possess a consistency ranging from soft solids to stiff liquids. These preparations are designed to deliver drugs locally to treat skin conditions, inflammation, infections, or pain, or systemically through transdermal absorption.

Types of Semisolid Dosage Forms

Ointments

Ointments are semi-solid preparations intended for external application to the skin or mucous membranes. They typically contain a high percentage of an oily or greasy base, such as petroleum jelly, lanolin, or mineral oil. Their primary function is to provide an occlusive film over the skin, which helps retain moisture and aid in the healing process. Because of their greasy nature, ointments are excellent for dry, scaly conditions but may be messy and stain clothing.

Creams

Creams are viscous emulsions (semisolid systems) of oil and water. They are generally classified as either water-in-oil (w/o) or oil-in-water (o/w) emulsions.

  • Oil-in-water creams: These are miscible with skin secretions and are easily washable with water. They feel less greasy than ointments and are cosmetically acceptable.
  • Water-in-oil creams: These are greasier and more moisturizing, suitable for very dry skin. They are not easily washed off with water.

Creams are often preferred over ointments for hairy areas and intertriginous zones (where skin folds rub together) because they are less occlusive.

Gels

Gels consist of a solid phase (the gelling agent) dispersed in a liquid vehicle, forming a semi-solid matrix. They can be aqueous or alcoholic. When applied, gels dry rapidly, leaving a residue that is often less greasy than creams or ointments. They are favored for their rapid absorption and cooling sensation, making them popular for muscle pain relief or acne treatment.

Pastes

Pastes are similar to ointments but contain a higher percentage of solid material, such as powders. This high solid content makes pastes stiff and absorptive. They are used to apply medication to small, circumscribed areas where protection and drying are needed. Pastes create a durable, protective layer that adheres well to the skin even in moist environments.

Advantages and Disadvantages of Semisolid Dosage Forms

Advantages:

  • Provide localized treatment at the site of application, minimizing systemic side effects.
  • Avoids first-pass metabolism in the liver when absorbed through the skin (transdermal delivery).
  • Moisturizing and protective properties (occlusive nature of ointments).
  • Easy to apply and generally well-tolerated by patients.

Disadvantages:

  • Can be messy to apply and may stain clothing or bedding.
  • Systemic absorption is variable and unpredictable compared to oral dosage forms.
  • Skin irritation or allergic reactions to base ingredients can occur.
  • Some preparations have an unpleasant odor or greasy feel.
