Who Can Enroll?
The program is open to adults (18+) who have a BMI of 27kg/m or higher, or who have a BMI of 25kg/m with obesityrelated health conditions. Special accommodations are available for:
- Patients with type 2 diabetes.
- Individuals with hypertension or dyslipidemia.
- Those recovering from bariatric surgery who need continued support.
Enrollment requires a referral from a primarycare physician within the Henry Ford network, though selfreferrals are also accepted for certain communitybased participants.
