Family Foundations Home Visiting Program

Supporting healthy families from the very start

Program Overview

The Family Foundations Home Visiting Program (FFHVP) is a federally funded initiative that pairs qualified families with trained home visitors. The aim is to improve pregnancy outcomes, strengthen parentchild relationships, and promote longterm health and educational success. Home visitors provide evidencebased guidance on nutrition, child development, stress management, and connecting families to community resources.

Since its inception, FFHVP has served thousands of families across urban, suburban, and rural settings, adapting its model to meet the cultural and linguistic needs of diverse populations.

Key Benefits for Families

  • Improved Maternal Health: Regular monitoring reduces complications and encourages healthy habits.
  • Enhanced Child Development: Early stimulation and responsive caregiving foster cognitive and social growth.
  • Reduced Parenting Stress: Emotional support and problemsolving strategies help parents feel confident.
  • Stronger Community Connections: Referrals to local services (e.g., mental health, housing, nutrition assistance).
  • LongTerm Economic Gains: Children who receive early support are less likely to need special education and more likely to graduate high school.

Eligibility Criteria

FFHVP is designed for families who meet one or more of the following conditions:

  • Pregnant women in their first or second trimester.
  • Families with infants up to 12 months of age at enrollment.
  • Lowincome households (often defined as 200% of the federal poverty level).
  • Firsttime parents or parents with limited support networks.
  • Families experiencing health disparities, including those in rural or underserved areas.

Eligibility is assessed by a local program coordinator, and enrollment is open yearround.

  • Higher scores on language and motor development assessments at 24 months.
