2. Apply the Tourniquet

Place the tourniquet approximately 3 to 4 inches above the intended puncture site to engorge the veins, making them more visible and palpable. Do not leave the tourniquet on for more than one minute to prevent hemoconcentration.

3. Select the Venipuncture Site

The antecubital fossa (the inner elbow area) is the most common site. The median cubital vein is preferred for its size and accessibility. Alternatives include the cephalic and basilic veins. Palpate to find a suitable, straight vein that is free from scarring, bruising, or infection.

Note: Avoid veins that feel hard or sclerosed, and do not use veins on the side of a mastectomy, with fistulas, or with compromised circulation.

4. Cleanse the Site

Using an alcohol prep pad or antiseptic wipe, clean the puncture site in a circular motion from the center outward. Allow the area to air dry completely to avoid contamination and stinging sensation for the patient.

5. Perform the Venipuncture

  1. Stabilize the vein by holding the patient's arm and anchoring the skin below the puncture site with your thumb.
  2. With the bevel of the needle facing upward, insert the needle into the vein at a 15 to 30 degree angle for vacutainer needles; a butterfly needle may require a smaller angle.
  3. Once blood flow is visible in the needle hub, attach the collection tube and allow it to fill.

6. Collect the Required Tubes in the Correct Order

Draw specimens in the recommended order of tube draw to prevent cross-contamination of additives between tubes. A typical sequence includes:

  • Blood culture tubes (sterile)
  • Citrate tubes (light blue top)
  • Serum tubes with or without clot activator (red or gold top)
  • Heparin tubes (green top)
  • EDTA tubes (lavender or pink top)
  • Oxalate/fluoride tubes (gray top)

Invert tubes gently the prescribed number of times to mix additives and blood properly without causing hemolysis.

7. Release the Tourniquet

Remove the tourniquet as soon as blood begins flowing into the first tube to minimize hemoconcentration and patient discomfort.

8. Withdraw the Needle and Apply Pressure

Once the last tube is filled, remove the needle smoothly and immediately apply gauze or cotton ball to the puncture site to stop bleeding. Ask the patient to apply gentle pressure and keep the arm extended.

9. Dispose of Needle Safely

Place the needle directly into an approved sharps container without recapping the needle to prevent accidental needle-stick injuries.

10. Label the Specimen Tubes

Clearly label each specimen tube with the patients information, date, time of collection, and collectors initials. Labeling should be done at the bedside immediately after collection to ensure accurate identification.

11. Observe the Patient

Check the puncture site for bleeding or swelling. Advise the patient to keep the pressure on the site and avoid heavy lifting with the arm for the next several hours.

Special Considerations

1. Pediatric Patients

Use smaller needles and appropriately sized tubes. Employ distraction and comfort techniques to reduce fear and pain. In some cases, capillary (finger or heel) puncture might be appropriate.

2. Patients with Difficult Veins

Veins that are small, fragile, or difficult to locate may require a butterfly needle, warming the arm, or ultrasound guidance. Avoid repeated punctures in the same site.

3. Fasting and Medication Instructions

Some blood tests require fasting or withholding specific medications beforehand. Always verify these instructions with the healthcare provider and inform the patient accordingly.

4. Infection Control

Maintain standard precautions to prevent cross-contamination. Handle all blood specimens as potentially infectious and follow institutional protocols for biohazard disposal.

Common Errors and How to Avoid Them

  • Hemolysis: Caused by rough handling, shaking tubes vigorously, or using a needle that is too small.
  • Incorrect Tube Order: Leads to contamination and inaccurate results. Follow the recommended order of draw.
  • Prolonged Tourniquet Application: May alter blood composition. Limit to less than one minute.
  • Poor Patient Identification: Can lead to specimen mix-up. Always verify identity multiple times.
  • Failure to Mix Tubes: Can result in clotting or incomplete anticoagulation in tubes containing additives.
  • Insufficient Blood Volume: May render tests invalid due to improper blood-to-additive ratio.
