Anti-M Blood Grouping Reagent
Introduction
Anti-M blood grouping reagent is a monoclonal antibody used in blood bank laboratories for the detection of the M antigen, which is part of the MNS blood group system. The MNS blood group system is one of the many human blood group systems that contains numerous antigens located on the surface of red blood cells. The M antigen was first discovered in 1927 and is now known to be encoded by the GYPA gene.
Composition and Properties
Anti-M reagent typically contains monoclonal IgM or IgG antibodies that react specifically with the M antigen on red blood cells. These reagents are usually supplied in liquid form and contain proteins, buffers, and preservatives that maintain their stability and reactivity. Most commercial Anti-M reagents are formulated to have low ionic strength to enhance antigen-antibody reactions.

- Hemolytic disease of the fetus and newborn (HDFN): Although less common than with other blood group systems, anti-M antibodies can occasionally cause HDFN.
- Autoimmune hemolytic anemia: Anti-M antibodies may be involved in certain cases of autoimmune hemolytic anemia.
Procedure for Using Anti-M Reagent
Sample Preparation:
- Obtain properly labeled EDTA anticoagulated blood samples.
- Prepare a 3-5% suspension of red blood cells in saline.
- Wash the red cells at least three times with saline to remove plasma proteins.
Testing Method:
- Label test tubes appropriately.
- Add 1 drop of Anti-M reagent to each test tube.
- Add 1 drop of red cell suspension.
- Mix gently and incubate at room temperature for 15 minutes.
- Centrifuge at 1000 g for 1 minute.
- Examine for agglutination macroscopically and, if necessary, microscopically.
- Record results according to established protocols.

Technical Information
The MNS blood group system is the second most complex after the Rh system, with over 46 antigens identified to date. The M antigen is part of this system and is encoded by the GYPA gene (glycophorin A). The GYPA gene has two major allelic forms: M and N, which differ by two amino acid substitutions at positions 1 and 5 of the glycophorin A protein. The M antigen is characterized by serine at position 1 and glycine at position 5, whereas the N antigen has leucine at position 1 and glutamic acid at position 5.
Storage and Stability
Proper storage of Anti-M reagent is crucial to maintain its effectiveness:
- Store at 2-8C in the original container.
- Protect from light and freezing.
- Do not use if the reagent appears turbid or shows signs of contamination.
- Following each use, ensure the container is tightly sealed to prevent contamination.
